Are you a Berkeley resident who donated to Syrak for Southside? Download the matching form here. There are two ways to fill it out:

• Print the form and sign it with a pen, the old fashioned way! Scan or take a picture of the filled-out form and email it to syrakforsouthside@gmail.com, with “Matching Form” and your first and last name in the subject line.

• Fill out the form digitally with a stylus or trackpad. You can NOT use a font or type to sign your signature! The signature must be handwritten, like with your finger on a laptop trackpad or touch screen. Other fields may be typed. Email the edited pdf to syrakforsouthside@gmail.com, with “Matching Form” and your first and last name in the subject line.

Remember: all other fields may be typed, but the signature MUST be digitally or physically HANDWRITTEN!
